|Windom Area Hospital Foundation Scholarship Applications Now Available|
I. Several $1000.00 scholarships will be presented to students expressing an interest in a healthcare profession. Students must have a grade point average of 3.0, OR be officially accepted into a healthcare program in order to be eligible for the scholarships. Scholarships are not tied to an employment commitment.
II. All applications must be neatly typed or printed in blue or black ink.
III. For those students who meet the criteria for Sanford Foundations scholarship, Windom Area Hospital will forward your application on to Sanford. If chosen, you would be eligible to receive an additional scholarship funded by Sanford. The Sanford Foundation scholarship is a separate process from the Windom Area Hospital Foundation scholarship, but only one application is required in order to be considered for both. The criteria for Sanford is described on the first page of the application. You MAY apply for the Windom scholarship, even if you do not qualify for the Sanford portion.
IV. In order to be considered for these scholarships, applicants need to:
A. Return the completed application on or before Friday, March 14th, 2014.
B. Provide an official copy of current transcript.
C. Provide proof of enrollment.
D. For those applicants who do not have previous work experience, you can detail your experiences with an athletic team, service group, or organization. (i.e. Boy Scouts, Girl Scouts, FFA, SADD, etc.)
E. Submit three (3) references from: 1) an instructor, 2) a supervisor, and 3) a coworker. If you do not have previous work experience, you may submit a reference from an administrator, coach, or an adult (non-relative) who is aware of your accomplishments and interest in the healthcare field.
F. Reapply each year you wish to be considered.
V. The completed application must be returned to the Windom Area Hospital Foundation, Attention Emily Masters, PO Box 339, Windom, MN 56101 on or before, Friday, March 14th, 2014.
VI. The Windom Area Hospital Foundation and the Sanford Foundation will make the final decision of the respective scholarship recipients. Both awards will be announced in April, with funds distributed in September.
VII. We are asking that those students who are selected for the scholarships commit to a group photograph of the recipients, which is scheduled for Wednesday, April 16th, at 5:15. Participating in the photo is strongly recommended, but is not a prerequisite for scholarship consideration.
