| Windom Area Hospital Foundation Scholarship Applications Now Available I. Several $1000.00 scholarships will be presented to students expressing an interest in a healthcare profession. Students must have a grade point average of 3.0, or be officially accepted into a healthcare program in order to be eligible for the scholarships.Students wishing to apply for the matching Sanford Foundation scholarship must meet BOTH of these requirements. Scholarships are not tied to an employment commitment. II. All applications must be neatly typed or printed in blue or black ink. III. For those students who meet the criteria for Sanford Foundation’s scholarship, Windom Area Hospital will forward your application on to Sanford. If chosen for a Sanford scholarship, you would be eligible to receive an additional $1,000. The Sanford Foundation scholarship is a separate process from the Windom Area Hospital Foundation scholarship, but only one application is required in order to be considered for both. The criteria for Sanford is described on the first page of the application. You MAY apply for the Windom scholarship, even if you don’t qualify for the Sanford portion. IV. In order to be considered for these scholarships, applicants need to: A. Return the completed application (below) by March 22nd, 2013. B. Provide proof of enrollment in a healthcare program. C. >Provide an official copy of current transcript. D. For those applicants who do not have previous work experience, you can detail your experiences with an athletic team, service group, or organization. (i.e. Boy Scouts, Girl Scouts, FFA, SADD, etc.) E. Submit three (3) references from: 1) an instructor, 2) a supervisor, and 3) a coworker. If you do not have previous work experience, you may submit a reference from an administrator, coach, or an adult (non-relative) who is aware of your accomplishments and interest in the healthcare field. V. The completed application (below) must be returned to the Windom Area Hospital Foundation, Attention Katie Slette, PO Box 339, Windom, MN 56101 no later than March 22, 2013. VI. The Windom Area Hospital Foundation and the Sanford Foundation will make the final decision of the respective scholarship recipients. The Windom awards will be announced in April.
